Frequently Asked Questions

  • For Individual Counseling we take:

    United Healthcare

    United HealthChoice

    Optum

  • To complete an assessment, you must first schedule an appointment, as all assessments are conducted one-on-one. The process takes approximately 1.5 to 2 hours, during which you will fill out necessary forms and answer several questions. You will then meet with a State Certified Facilitator to discuss your past or present arrest. Based on this evaluation, the facilitator will provide you with referrals and recommendations that you will need to complete.

  • To get started, give us a call at 405-839-7600 or fill out the contact form on our website. We’ll discuss your needs, help you choose the right service or course, and set up your appointment date.

  • Yes. A Drug & Alcohol (ADSAC) Assessment must be completed before you can enroll in any DUI course. We’ll provide you with details on how to complete the assessment and bring proof of completion to class.

  • Absolutely. Most of our counseling sessions and ADSAC courses are available via telehealth, allowing you to participate from home or another convenient location.

  • If you are more than 15 minutes late, you won’t be admitted to that session and must make it up in the following weeks—within the time limits for course completion.

  • For the ADSAC Courses and Assessment, we accept exact cash, debit cards, and credit cards. We are in-network with United HealthCare, HealthChoice, Oscar and Optum for Individual counseling. Payment plans are also available for certain courses—please inquire for details.

  • It varies by course. For example, our 10-Hour DUI/ADSAC Course must be completed within 60 days, while the 24-Hour ADSAC Course has a 180-day completion window. Specific timelines will be provided when you enroll.

  • Unfortunately, if you exceed the time limit and have not completed all required sessions, you must restart the course from the beginning, and any previous fees are non-refundable.

  • Our ADSAC assessments and courses are designed to help individuals work toward reinstating their driver’s licenses. Follow the steps required by your state’s licensing authority, and we will provide any necessary documentation once you meet course requirements.

  • Feel free to contact us through our website’s form, via email at info@maplelifesolutions.com, or call us at 405-839-7600. We’re happy to discuss your specific situation and answer any further questions.
